Help us get to know you by filling in this form to the best of your abilities. Please only submit an application when you and all household members have agreed on welcoming a new dog into the family. We are a small rescue and our dogs are our family. We take adoption seriously and want to ensure that all our dogs are going to their forever homes. Once you are approved to adopt there is a contract to be signed and adoption/transportation costs are due.

Agreement and Acknowledgement

I understand that there is a $150 adoption fee.

I understand that this fee does not cover the full costs of rescuing and caring for the animal.

I understand that this fee does not cover transportation of the dog.

I understand that adopting a dog is a long term commitment of up to 10+ years.

I understand that owning a dog means budgeting for food, preventative medicine, occasional veterinary visits and emergencies.
